WHAT TO EXPECT

New Patient Appointment
Your first appointment may take up to one hour. We ask that you bring in your current insurance card, a valid id and any discs containing previous radiology studies. We are particularly interested in the CT scan and MRI’s that pertain to the reason for your visit. Results of evaluations performed by other doctors are also helpful. Download a Record Release Authorization Form.
We ask that you arrive 30 minutes prior to your scheduled appointment time to allow adequate time to fill out your New Patient Paperwork and to allow time for our staff to input your information into our system for your provider to review. Download New Patient Paperwork. You can start completing these forms at home. Filling out the forms ahead of time is not only more efficient but it gives you time to think about your symptoms before your visit. This will help you to best explain your problem to your provider.
