It is very important to come prepared for your appointment, which includes providing information about your medical history. We obtain some of this information from the forms we ask you to fill out. Filling out the forms ahead of time is not only more efficient but gives you time to think about your symptoms before your visit. These forms can be downloaded from the list on the left. All patients need to complete the Consent for Treatment Form, the Privacy Notice and the Referral Guideline Form. In addition to this, patients scheduled for an EMG need to complete the EMG Registration Form and patients scheduled for a consultation need to complete the Registration and Patient Questionnaire.

Bringing MRI and CT scan films is also very important, (most are available now on CD type disks), as is bringing the results of evaluations done by other doctors. Click Here to Download a Record Release Authorization.

All patients scheduled for an EMG should not apply skin creams before coming for the procedure. The creams make it more difficult for the recording electrodes to stick to your skin. Click here to learn more about the EMG procedure.

The vast majority of our patients are referrals from other physicians and it is to them that we communicate the results of our evaluations. We do consultations that entail taking a medical history and doing a neurological examination and EMG nerve conduction study that require the use of a machine to evaluate the peripheral nerves and muscles. In addition to our busy office practice, we provide comprehensive hospital care. We are an integral part of The Stroke Center at Hartford Hospital and provide emergency coverage during the day as well as the night. We are also committed to providing services in our suburban locations in Bloomfield and Glastonbury, though due to our hospital obligations we cannot staff each suburban office every day.

 

Prescriptions called into the office by 3 pm will be called into your pharmacy that day. They will otherwise be refilled by noon the next day. Please give the number of your pharmacy and, most importantly, the dosage of the medication you are requesting and how you are taking it.

To refill your prescription call (860) 522-4429 ext 35. Please spell your name and state your date of birth. Be sure to give the telephone number of your pharmacy.

 

We encourage you to contact your doctor by phone if you have a medical question or a symptom that you wish to discuss. We will make every effort to return your call the same day. The secretaries are listed below with their extensions. Many insurance companies require their members to obtain a referral from their primary care physicians in order to be treated by a specialist.
